All current medications in a ziplock bag
Please arrive at least 15 minutes prior to your first appointment.
Copies of any records from doctor’s related to your visit with us

Our relationship with you is built upon the sharing of knowledge, confidence and trust. We ask that you voice your questions and concerns. Maintaining your privacy is important to us. Everything about your visit is held in strict confidence. With communication and understanding, we can help you maintain your optimum health.

Insurance


The Bridge Medical accepts payment from a number of medical insurance plans. If we are a participating provider on your plan, we will accept your valid insurance card as payment for the services provider to you. If a co payment, deductible or coinsurance applies, you will be required to pay this at the time of service.
We will also file a claim on your behalf directly to your insurance company. Since insurance benefits (including copayments, deductibles, and co insurance amounts) billing addresses and eligibility change frequently for our patients, we rely on the information printed on your insurance card to file your claim properly.
